Form Issue Item "Other" Fixed

We apologize if it took some time to find the culprit on the request form (same goes with the other sites) - it should be working now on this site. You can now request and choose the option "Other" in the list without issues. please let us know if you still experience any problems.   We appreciate your patience in this. Thank you very much.   PS: we'll be implementing a ticket form this week to organize the issues in the site.
